Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapist for an in-house Skilled Nursing Facility in Sutherland, NE. The PRN role focuses on providing high-quality therapy services while adhering to CMS and facility standards.

The position requires a BS in Physical Therapy, an active PT license, and commitment to ongoing education. You will collaborate with an interdisciplinary team and manage care plans and documentation in NetHealth EMR.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Ability to complete CMS documentation in NetHealth EMR.

Responsibilities

• Provide physical therapy services to residents per established standards and procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Evaluate residents to determine need for intervention and plan treatment.
• Develop and implement treatment plans with short and long-term goals.
• Communicate evaluation findings and recommendations to residents and care teams.
• Complete compliant documentation including CMS expectations in NetHealth EMR.
