Job summary

Evolve Therapy Services is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Ohio. The role is part time or PRN, assisting the Physical Therapist with resident care and treatment planning.

You will conduct screenings, participate in both individual and group sessions, and document progress in NetHealth EMR while coordinating with the PT and care team to meet CMS expectations and resident goals.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in the evaluation under the direction of the Physical Therapist.
• Assist the Physical Therapist in identifying a target list with short- and long-term goals and treatment plans.
• Conduct individual and group treatments as outlined in the treatment plan, including strength, mobility, balance, gait, and ROM activities.
• Assist in selecting PT techniques/media and adapt as indicated by resident performance after consultation with the PT.
• Monitor residents’ response to intervention and consult with the PT if modification is indicated.
• Help establish and modify resident schedules with the PT and team members.
• Adhere to department and resident schedules and adjust as needed for treatment regimens.
• Train resident, family and staff in the use of equipment.
• Participate in resident, family, and staff education.
• Document services and progress in CMS-compliant records (NetHealth EMR).
• Contribute to Quality Assurance programs as required.

Education
