Job summary

Qualifications

• Licensed PTA in Illinois (IDFPR)
• BLS within 30 days - American Heart Association
• Continue education as required for acute stroke care and program certifications

Responsibilities

• Educates patients, family, caregivers, students and other health care providers using effective teaching methods.
• Follows the therapist's plan of care and adjusts treatment based on patient response and evidence after communication with the evaluating therapist.
• Produces documentation per Clinical Documentation Policy.
• Completes annual education to comply with DNV Hip and Knee Replacement requirements (when applicable).
• Implements intervention plans according to the Physical Therapist's plan of care.
• Uses standardized outcome measures to reflect functional performance.
• Completes documentation per regulatory standards and organizational policies.
• Collaborates with patient and family throughout rehabilitation and provides education/training as needed.
• Communicates with supervising Occupational Therapist about changes in patient status or plan of care.
• Communicates discharge and equipment needs to the medical team.
