Job summary

PruittHealth @ Home is seeking a Physical Therapist Assistant (PTA) to provide qualified home health services under the supervision of a physical therapist in patients' residences in the Charleston/Dorchester area. You will assist with developing and implementing the plan of care, collaborate with the interdisciplinary team, maintain licenses and records, and meet productivity standards.

Qualifications

• Must have a valid and unrestricted professional license in state of practice.
• Graduate of an accredited Physical Therapy Assistant program.
• If foreign-trained, the program must be deemed substantially equivalent to a PTA entry-level education in the U.S. by a credentials evaluation organization approved by the APTA.

Responsibilities

• Develop and implement plan of care within goals defined by the physician, patient/patient family and interdisciplinary team.
• Collaborate with interdisciplinary team to develop, integrate, and manage the patient care plan of care.
• Demonstrate competency in home care and specific patient care skills as required for provision of care.
• Meet agency productivity standards and utilize time/resources effectively.
• Complete documentation timely, accurately, and at the point of care, according to industry standards.
• Assist the physical therapist in assessing the patient’s disability and level of function.
• Maintain current license, certification, and clinical records to meet regulations.
• Participate in surveys and required reports.
• Report any alleged or suspected violations to supervisor until resolved.
• Be self-directed, prioritize work, and meet deadlines.
