Job summary

PruittHealth @ Home is hiring a Physical Therapy Assistant (PTA) to provide in-home physical therapy to patients in the Charleston/Dorchester area. This full-time role supports care plans under a supervising PT and emphasizes high-quality patient outcomes in home settings.

Qualifications

• Must have a valid and unrestricted professional license in state of practice.
• Graduate of an accredited Physical Therapy Assistant program.
• Foreign-trained graduates must have credentials evaluated for equivalence by APTA-approved organization.
• Current CPR certification.

Responsibilities

• Assist with the development and implementation of the plan of care within goals defined by the physician, patient/patient family and interdisciplinary team.
• Help maintain continuity of care by collaborating with the interdisciplinary team and attending physician to develop, integrate, and manage the patient care plan.
• Demonstrate intermediate knowledge of home care and competency in discipline for specific patient care skills.
• Meet agency productivity standards and utilize time and resources effectively.
• Complete documentation timely, accurately, at the point of care, according to industry standards.
• Assist the physical therapist in assessing the physical disability and level of function of patients.
• Maintain current license, certification, and clinical records to meet regulations and guidelines.
• Participate in center/agency surveys and required reports.
• Report any alleged or suspected violations to supervisor until resolved.
• Be self-directed, prioritize, manage time, and meet outcomes and goals by deadlines.
