• Minimum of 2 years' therapy experience.
• Valid driver’s license and automobile liability insurance.

Responsibilities

• Assist with development and implementation of the plan of care within goals defined by physician, patient/family and interdisciplinary team.
• Collaborate with interdisciplinary team and attending physician to develop and manage the patient care plan of care.
• Demonstrate competency in home care and specific patient care skills required for provision of care.
• Meet agency productivity standards and utilize time/resources effectively.
• Complete documentation timely and accurately at the point of care according to industry standards.
• Assist the physical therapist in assessing the patient’s physical disability and level of function.
