Job summary

PruittHealth is seeking a Physical Therapy Assistant (PTA) for Home Health services to provide therapy in patients’ homes under PT supervision. The role emphasizes collaboration with the interdisciplinary team to implement the plan of care and meet approved outcomes.

Responsibilities include advancing the patient’s functional goals, timely documentation, and adherence to agency standards. Licensure and a valid driver’s license are required, with preferred home health experience.

Qualifications

• Minimum of 2 years therapy experience in home health or related setting.
• Valid driver’s license and automobile liability insurance.
• Graduate of an accredited Physical Therapy Assistant program; foreign-trained must be evaluated for U.S. equivalence.

Responsibilities

• Assist development and implementation of the plan of care within goals defined by physician, patient/family and interdisciplinary team.
• Collaborate with interdisciplinary team and attending physician to develop and manage the plan of care.
• Demonstrate competency in home care and specify patient care skills for provision of care.
• Meet agency productivity standards and use time/resources efficiently.
• Complete documentation timely and accurately at the point of care.
• Assess physical disability and functional level of patients with PT supervision.
• Maintain current license, certification, and clinical records to meet regulations.
• Participate in licensure surveys and required reports.
• Report any violations to supervisor promptly.
• Be self-directed, prioritize, manage time, and meet deadlines.
