Job summary

Trilogy Home Healthcare, a Humana company, is hiring a Certified Physical Therapy Assistant to provide one-on-one PTA services in patients' homes across Hernando County, FL. This PRN/per diem role offers flexible scheduling and the autonomy to support patients’ independence and quality of life.

The ideal candidate holds a Florida PTA license, has at least 1 year of experience (home health preferred), and can follow physician orders under a supervising PT.

Qualifications

• Current Physical Therapy Assistant (PTA) license/certification required.
• Valid Florida driver's license, active vehicle insurance, and clear MVR.
• 1+ year of experience required, Home Health experience preferred.
• Sound judgement and a passion for patient care and outcomes.

Responsibilities

• Provide one-on-one physical therapy services in a home health setting.
• Follow physician orders and treatment plans under the supervision of a Registered Physical Therapist (PT).
• Deliver therapy that helps patients achieve independence and improve their quality of life.
