Job summary

Trilogy Home Healthcare in Polk County, FL is seeking a Certified Physical Therapy Assistant (PTA) to join our home health team on a PRN/per diem basis. This role provides one-on-one physical therapy in patients' homes, following physician orders under the supervision of a Registered Physical Therapist to improve independence and quality of life.

Qualifications

• Current Physical Therapy Assistant (PTA) license/certification required.
• Valid Florida driver's license, active vehicle insurance, and clear MVR.
• 1+ year of experience required, Home Health experience preferred.
• Sound judgement and a passion for patient care and outcomes.

Responsibilities

• Provide one-on-one physical therapy services in a home health setting.
• Follow physician orders and treatment plans under the supervision of a Registered Physical Therapist (PT).
