Job summary

Trilogy Home Healthcare, a Humana company, is seeking a dedicated Certified Physical Therapy Assistant to join our home health team in Seminole County, FL. This PRN/Per Diem position offers flexible scheduling and clinical autonomy in a supportive, award-winning environment.

Applicants must hold a PTA license, a Florida driver’s license with valid insurance, and have 1+ year of experience (Home Health preferred). Lymphedema certification is a plus as we focus on patient outcomes.

Qualifications

• Current Physical Therapy Assistant (PTA) license/certification required.
• Valid Florida driver’s license with active vehicle insurance and clear MVR.
• 1+ year of experience; Home Health experience preferred.
• Lymphedema Certification preferred.

Responsibilities

• Provide one-on-one physical therapy services in a home health setting.
• Follow physician orders and treatment plans under supervision of a Registered Physical Therapist (PT).
• Deliver therapy that helps patients achieve independence and improve quality of life.
