Job summary

Horizon Home Health Care is seeking a dedicated Physical Therapist Assistant (PTA) to join our home health team in Greene County, Ohio and surrounding areas. This part-time/PRN role works under a licensed PT to help patients regain strength and independence in their own homes.

You will implement care plans, guide patients through therapeutic exercises, educate caregivers, document progress, and collaborate with a multidisciplinary team to ensure safe, coordinated care.

Qualifications

• Current Ohio PTA license is required.
• CPR and/or BLS certification is required.
• Valid driver’s license and reliable transportation are required.
• Strong communication, organizational and interpersonal skills are essential.
• Home health experience is preferred.

Responsibilities

• Provides physical therapy treatments under the supervision of a licensed PT.
• Assists patients with exercises, gait training, balance activities, transfers and mobility techniques.
• Monitors progress and communicates changes to supervising PT and care team.
• Educates patients and caregivers on home exercise programs, fall prevention, and safe mobility techniques.
• Completes accurate and timely clinical documentation.
• Collaborates with physicians, nurses, OT, SLP and other healthcare professionals for coordinated care.
• Maintains compliance with agency policies and state/federal home health regulations.
