Job summary

Horizon Health Network is seeking a dedicated Physical Therapist Assistant (PTA) for home health in Greene County, Ohio. You will work under the supervision of a licensed PT to provide individualized therapy in patients' homes, helping them regain mobility, reduce pain, and achieve independence.

Responsibilities include guiding therapy sessions, documenting progress, educating patients and caregivers, and coordinating with the care team to ensure safe, effective care.

Qualifications

• Ohio PTA license is required.
• CPR/BLS certification required.
• Valid driver's license and reliable transportation.

Responsibilities

• Provides physical therapy treatments under the supervision of a licensed PT.
• Assists patients with therapeutic exercises, gait, balance, and mobility training.
• Monitors progress and communicates changes to the supervising PT and care team.
• Educates patients and caregivers on home exercise programs and safety.
• Completes accurate clinical documentation and follows regulations.
• Collaborates with physicians, nurses, and therapists to coordinate care.
