Job summary

Horizon Health Network is seeking a dedicated Physical Therapist Assistant (PTA) to join our home health team in the Piqua area. Under the supervision of a PT, you will help patients regain mobility, reduce pain, and regain independence in their homes.

The role emphasizes individualized therapy, compassionate care, and collaboration with an interdisciplinary team across Miami County, Ohio and surrounding areas. Part-time flexibility and patient-centered scheduling are offered.

Qualifications

• Ohio PTA license required.
• CPR/BLS certification required.
• Home health experience preferred but not required.

Responsibilities

• Administers physical therapy treatments under the supervision of a licensed PT.
• Assists patients with therapeutic exercises, gait training, balance, transfers and mobility.
• Monitors progress and communicates changes to supervising PT and care team.
• Educates patients and caregivers on home exercise programs, fall prevention, and safe mobility.
• Completes accurate and timely clinical documentation.
• Collaborates with physicians, nurses and therapists to ensure coordinated care.
• Maintains compliance with agency policies and home health regulations.

We are looking for a dedicated and compassionate PTA to join our growing home health team. Working under the supervision of a PT, you will provide individualized therapy services that helps patients improve mobility, reduce pain, and safely regain independence in the comfort of their own homes. Responsibilities

• Provides physical therapy treatments according to the established plan of care under the direction of a licensed PT
• Assists patients with therapeutic exercises, gait training, balance activities, transfer training and mobility techniques
• Monitors patient progress and communicate changes in condition to the supervising PT and care team
• Educates patients and caregivers on home exercise programs, fall preventions, and safe mobility techniques.
• Completes accurate and timely clinical documentation
• Collaborates with physicians, nurses, occupational therapists, speech-language therapists and other healthcare professionals to ensure coordinated patient care
• Maintains compliance with agency policies and state and federal home health regulations

Qualifications

• Current Ohio Physical Therapist Assistant license
• Current CPR and/or BLS certification
• Valid driver's license, reliable transportation and proof of auto insurance
• Strong communication, organizational and interpersonal skills
• Home health experience is preferred, but not required
