Boost Home Healthcare is seeking a dedicated Physical Therapist Assistant to deliver physician-prescribed PT in patients’ homes, with intermittent visits and supervision from clinical leadership.

Qualifications

• Must graduate from an accredited PTA program approved by the American Physical Therapy Association.
• Currently licensed as a Physical Therapist Assistant in the practicing state.
• Minimum six months PTA experience; two years in home health preferred.
• Familiarity with Medicare OASIS documentation.
• Valid driver’s license and reliable transportation.
• Residence in Lorain County (Avon, Elyria, Lorain) is ideal.

Responsibilities

• Provide physician prescribed physical therapy.
• Improve or minimize residual physical disabilities of the patient.
• Return the individual to optimum and productive level within the patient’s capability.
• Participate with all other home care personnel in patient care planning.
• Documents all services provided, observations, client progress, and recommendations.
• Participated in interdisciplinary activities per Agency guidelines.

• Licensed driver with automobile insured in accordance with state/Agency requirements. Reliable transportation with good driving record.
• Ideal candidate resides in Lorain County (Avon, Elyria, Lorain, etc.)
