Job summary

Horizon Home Health Care is seeking a dedicated Physical Therapist Assistant (PTA) to join our home health team in Ohio. Working under a PT, you will deliver individualized therapy in patients’ homes to improve mobility and independence.

You will educate patients and caregivers, monitor progress, and document care while collaborating with a broader care team. This role offers part-time or PRN hours with flexibility and a supportive environment.

Qualifications

• Must hold an Ohio PTA license.
• CPR/BLS certification required.
• Valid driver's license with reliable transportation.
• Strong communication and interpersonal skills.
• Home health experience preferred.

Responsibilities

• Provides physical therapy treatments under the supervision of a licensed PT per plan of care.
• Assists patients with therapeutic exercises, gait training, balance activities, transfer training and mobility techniques.
• Monitors patient progress and communicates changes to the supervising PT and care team.
• Educates patients and caregivers on home exercise programs, fall prevention, and safe mobility techniques.
• Completes accurate and timely clinical documentation.
• Collaborates with physicians, nurses, OT, SLP and other professionals for coordinated care.
• Maintains compliance with agency policies and state and federal home health regulations.
