Job summary

Redcrest Home Health LLC is seeking a Physical Therapy Assistant (PTA) to provide comprehensive skilled services in patients' homes under the supervision of a Physical Therapist. The PTA will educate patients and families, ensure safety, and maintain accurate documentation in line with Medicare/Federal guidelines.

The role requires at least one year of PTA experience in an acute setting; home health experience is preferred.

Qualifications

• Minimum of one year of PTA experience in an acute setting.
• Home care experience is desired.
• Ability to communicate effectively with patients, families, physicians and co-workers.
• Knowledgeable on therapy best practices.
• Ability to make appropriate physical therapy judgments.
• Detail-oriented and observant.
• Disciplined work ethic with timely delivery.
• Valid driver’s license and auto insurance with your name listed as a driver.

Responsibilities

• Provides skilled PTA services under the direction of a Physical Therapist.
• Educates patients and family members while ensuring safety.
• Documents treatment, assessments and patient education.
• Completes chart audits as requested and ensures timely documentation.
• Communicates changes to scheduler and participates in case conferences.
• Drives to patient locations and attends assigned visits.
• Assists or performs other duties within restrictions as assigned.
