Job summary

Redcrest Home Health LLC is seeking a licensed Physical Therapist Assistant to provide skilled services in patients’ homes under the direction of a Physical Therapist. The role emphasizes accurate documentation, safety, and adherence to Medicare/Federal guidelines, with a focus on patient and family education and recovery goals.

Candidates should have at least one year of PTA experience in an acute setting and ideally prior home care exposure.

Qualifications

• Active Physical Therapist Assistant license in the state of employment.
• Minimum of one year of PTA experience in an acute setting is required.
• Home care experience is desired.
• Ability to communicate effectively with patients, families, physicians and coworkers.
• Knowledge of therapy best practices.
• Ability to make appropriate physical therapy judgments.

Responsibilities

• Provides skilled services in a patient’s home as developed by the Physical Therapist plan of care.
• Observes, records, and reports patient reactions, changes, and progress.
• Performs physical therapy procedures as prescribed.
• Treats patients to relieve pain and restore function using therapeutic modalities.
• Educates patients and families on therapy techniques and safety.
