Job summary

Evolve Therapy Services, LLC is seeking a compassionate Physical Therapist for an in-house Skilled Nursing Facility in Ponca City, OK. This PRN position supports resident care by planning, directing, and providing physical therapy services in accordance with federal, state, and local standards.

Responsibilities include patient evaluations, treatment planning, documentation, and collaboration with an interdisciplinary team.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and departmental procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Evaluate residents to obtain data for treatment planning and implementation; develop short- and long-term goals.
• Develop treatment plans and communicate findings to residents, families, and interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR systems.
