Job summary

Elite Home Health, part of the LHC Group and under the Optum family, is hiring a Physical Therapist Assistant to provide plan-of-care-based therapy in patients' homes. This role emphasizes care, connection with families, and community integration.

You will follow a supervising PT's plan, lead patient education, and perform necessary therapeutic activities to improve patient well-being. A sign-on bonus is available.

Qualifications

• Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice.

Responsibilities

• Follow state-specific laws governing home care PT services and treat only within the plan of care.
• Instructs patients in active and passive exercise, gait, and mobility training.
• Observe and report patient responses to treatment and coordinate care with the healthcare team.
• Educate patients and families on therapy within the PTA scope.
