Job summary

Adaptive Home Health, LLC is seeking a Physical Therapy Assistant (PTA) to deliver skilled home visits in Detroit, MI, under the supervising PT’s plan of care. The role emphasizes functional mobility, gait and balance, transfers, and home safety.

We offer a full-time field-based position with flexible scheduling and an AI-native platform to streamline charting and documentation, plus a $75k-$100k/year compensation with productivity-based points.

Qualifications

• Active Michigan PTA license in good standing.
• Reliable transportation and valid driver’s license.
• Home health experience strongly preferred.
• Able to carry/transport basic therapy equipment.
• Competency in core home health PTA scope: gait, transfers, balance, safety.

Responsibilities

• Deliver PTA visits in the home under the supervising PT’s plan of care.
• Execute treatment plans with a focus on functional mobility and measurable progress.
• Set/track objective progress with supervising PT.
• Drive multidisciplinary coordination and keep the care team moving.
• Coach patients and caregivers with practical home programs.
• Document promptly and thoroughly.
