Job summary

Adaptive Home Health in Detroit is seeking a Physical Therapy Assistant to deliver skilled PTA visits in patients' homes under the supervising PT’s plan of care. The role emphasizes functional mobility, gait and balance, transfers, and home safety, with a focus on high-quality patient outcomes.

We offer flexible full-time scheduling, a productivity-based compensation model ($58 per point, target ~30 points/week), and a tech-enabled platform that streamlines charting and post-visit documentation.

Qualifications

• Active Michigan PTA license in good standing.
• Reliable transportation and valid driver’s license.
• Home health experience strongly preferred.
• Able to carry/transport basic therapy equipment as needed.

Responsibilities

• Deliver PTA visits in the home under the supervising PT’s plan of care.
• Execute treatment plans with focus on gait/balance, transfers, strengthening, functional mobility, home safety.
• Set/track progress and communicate with supervising PT.
• Drive multidisciplinary coordination and coach patients and caregivers.
• Document promptly and thoroughly with defensible, efficient charting.
