Adaptive Home Health, LLC is hiring a Physical Therapy Assistant (PTA) in Detroit, field-based. You will deliver skilled PTA visits under the supervising PT's plan of care, focusing on gait/balance, transfers, and home safety to maximize patient outcomes.

Qualifications

• Active Michigan PTA license (in good standing).
• Reliable transportation, valid driver's license, auto insurance.
• Home health experience strongly preferred.
• Able to carry/transport basic therapy equipment as needed.
• Competency in core home health PTA scope.

Responsibilities

• Deliver skilled PTA visits in the home under the supervising PT's plan of care.
• Execute treatment plans with focus on gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.
• Set/track objective progress within PTA scope and communicate updates to supervising PT.
• Drive multidisciplinary coordination with care team and referral partners.
