Qualifications

• Master's Degree in Social Work required.
• Experience in individual and family assessment, crisis intervention, grief and loss counseling, and discharge planning preferred.
• MI license or licensure preferred for MSW practice.

Responsibilities

• Collaborate with an interdisciplinary team to assist patients and families with complex psychosocial needs.
• Conduct psychosocial assessments and document plans in the medical record.
• Communicate with alternate sites of care and community agencies to address patient needs.
• Maintain knowledge of community resources and regulations relevant to the role.
• Participate in department or regional committees as requested.
