Corewell Health is seeking a social work professional to support patients and families with complex psychosocial needs. You will collaborate with an interdisciplinary team and coordinate discharge planning across care settings, providing timely referrals and resources.

Responsibilities include conducting psychosocial assessments, documenting plans in the EMR, and offering guidance to the care team, while maintaining up-to-date community resource knowledge.

Qualifications

• Master's degree in Social Work is required.
• Experience in individual and family assessment, crisis intervention, grief and loss counseling, and discharge planning preferred.
• MI State licensure for Master Social Work is required upon hire.

Responsabilités

• Collaborate with an interdisciplinary team to assist patients and families with psychosocial needs.
• Coordinate with alternate sites of care and community agencies for discharge planning.
• Document assessments and plans in the electronic medical record reflecting current condition and transition needs.
• Provide consultation and resources to healthcare team and stay current on community resources.
