Bakersfield Post Acute in Bakersfield, CA is seeking a dedicated Physical Therapist (PT) for PRN/per diem coverage in our skilled nursing facility and short-term rehab setting. You will evaluate and treat residents, develop individualized plans, and collaborate with an interdisciplinary rehab team.

Qualifications

• Education & Experience: Bachelor’s degree in Physical Therapy required. Master’s or Doctorate in Physical Therapy preferred.
• Licensure: California Physical Therapist license in good standing; continue ed requirements.
• Skills: Proficient in electronic documentation systems; strong clinical judgment, communication, and organization.

Responsibilities

• Evaluate residents within 24 hours (or one business day) of physician referral and document delays.
• Develop individualized treatment plans and obtain physician approval.
• Treat residents according to the physician-approved plan of care.
• Supervise PTAs, aides, and students per state act.
• Communicate resident progress with rehab and interdisciplinary teams.
• Complete evaluations and discharge documentation per regulatory and payer requirements.
• Educate residents and families on exercises, mobility, safety, and discharge planning.
• Participate in discharge planning and coordination of care.
