Job summary

Civita Care Center at Cheshire in Cheshire, CT is seeking a Physical Therapist (PT) on a PRN basis. You will evaluate residents, develop individualized plans of care, and provide skilled interventions to help regain mobility and independence.

You'll work with an interdisciplinary team to set rehabilitation goals, track progress, educate residents and caregivers, and maintain timely documentation. A Connecticut PT license and graduation from an accredited PT program are required; new graduates

Qualifications

• Active Connecticut Physical Therapist license required.
• Graduate from accredited Physical Therapy program.
• Strong clinical, communication, and organizational skills.

Responsibilities

• Complete physical therapy evaluations and develop individualized plans of care.
• Provide interventions addressing strength, balance, gait, transfers, endurance, and mobility.
• Establish rehabilitation goals and track resident progress.
• Adjust treatment plans based on clinical findings and response to therapy.
• Educate residents and caregivers on therapeutic exercises and safe mobility strategies.
• Maintain clear and timely clinical documentation and coordinate with PTAs and nursing.

Civita Care Center at Cheshire | Cheshire, CT

$55–$60/hour | PRN

We are hiring a Physical Therapist (PT) on a PRN basis at Civita Care Center at Cheshire. Join a collaborative rehabilitation setting where you can provide individualized therapy focused on helping residents regain mobility, build strength, and work toward greater functional independence.

What You'll Do

• Complete physical therapy evaluations and develop individualized plans of care


• Provide skilled interventions addressing strength, balance, gait, transfers, endurance, and mobility


• Establish appropriate rehabilitation goals and track resident progress


• Adjust treatment plans based on clinical findings and response to therapy


• Educate residents and caregivers on therapeutic exercises and safe mobility strategies


• Maintain clear and timely clinical documentation


• Coordinate care with PTAs, nursing, and other members of the interdisciplinary team
