$55-$60/hour | PRN

Stern Therapy Consultants is hiring a PRN Physical Therapist at Civita Care Center Bayview in Waterford, CT. Help skilled nursing residents work toward greater independence by addressing the physical challenges that affect getting out of bed, moving between surfaces, and walking safely.

What You'll Do

• Evaluate functional mobility, strength, balance, and range of motion, taking medical history and precautions into account.
• Set measurable rehabilitation goals and establish treatment plans suited to each resident's needs.
• Provide therapeutic exercise, gait training, and functional activities that support daily movement.
• Assess mobility equipment needs and instruct residents and caregivers in safe use.
• Review treatment outcomes, modify care plans, and oversee PTA participation in resident services.
• Complete clinical documentation and contribute mobility recommendations to interdisciplinary care and discharge planning.
