Job summary

Continuum Therapy Partners is seeking a qualified Physical Therapist to join our growing team in Colby, WI at Colonial Health Services. A Doctor of Physical Therapy (DPT) and a current, unencumbered state license are required, with NPTE passage.

PRN daytime schedule offers flexibility and clinical collaboration to improve patient outcomes. Ideal candidates have 1 year of PT experience and are committed to educating patients and families on home exercise programs and injury prevention while

• Current, unencumbered state Physical Therapy License; NPTE must be passed.
• 1 year of previous experience as a Physical Therapist preferred.
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
