Job summary

Continuum Therapy Partners is seeking a Physical Therapist to join as a PRN provider in Orange Park, FL. The role supports flexible daytime coverage at Oak View Health and Rehabilitation Center, with patients receiving care shaped by experienced clinicians.

Candidates must hold a DPT, have a current state PT license, and NPTE eligibility. Prior PT experience is preferred, and the role starts 1/1/27 to meet scheduling needs.

• Evaluates and develops individualized treatment plans.
• Documents progress and educates patients.

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
