Job summary

Continuum Therapy Partners, LLC is seeking a Physical Therapist to join our PRN team serving Hayward, WI. You’ll provide high-quality care with the same standards as our full-time staff and choose your daytime hours to fit your schedule.

Requirements include a Doctor of Physical Therapy, an active state license, NPTE completion, and a preferred 1 year of PT experience. You’ll evaluate and adjust treatment plans, document progress, and educate patients and families on home exercise programs.

Qualifications

• Current, unencumbered state Physical Therapy License; must pass the NPTE.
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities
