Job summary

Continuum Therapy Partners, LLC seeks a PRN Physical Therapist to help maintain continuity of care with the same clinical standards as our full-time team.

You will choose your schedule, working daytime shifts at Spruce Manor Nursing & Rehabilitation Center in West Reading, PA. You’ll collaborate with a leadership team of therapists and support patient-centered outcomes with flexible, quality care.

Qualifications

• Doctor of Physical Therapy (DPT) degree required.
• Current, unencumbered state Physical Therapy License; NPTE required.
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
