Continuum Therapy Partners is seeking a Physical Therapist for PRN coverage in Hayward, WI. You will deliver high-quality care with the same standards as our full-time team, choosing when you work while receiving support from a leadership team of experienced therapists.

Qualifications

• DPT degree required.
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
