Continuum Therapy Partners, LLC seeks a Physical Therapist to join our PRN team in Hayward, WI. You choose when you work and deliver care with the same clinical standards and voice as our full‑time therapists.

Requirements include a Doctor of Physical Therapy (DPT), a current Wisconsin PT license (NPTE passed), and one year of PT experience preferred. You will evaluate, develop, and implement individualized plans, document progress, and educate patients and families on home exercises and injury

Qualifications

• DPT required.
• Current, unencumbered state Physical Therapy License; must pass NPTE.
• 1 year of previous experience as a Physical Therapist preferred.
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities

• Doctor of Physical Therapy (DPT) degree required
