Job summary

Continuum Therapy Partners in Monroe, WI seeks a part-time Physical Therapist to join a patient-centered network focused on quality care. You will work daytime at Monroe Manor Nursing & Rehab Center, delivering compassionate therapies.

You will evaluate, develop, and implement individualized treatment plans, document progress, and adjust care based on outcomes. You will educate patients and families on home exercise programs and injury prevention, coordinating with other disciplines to optimize

Qualifications

• Doctor of Physical Therapy (DPT) degree required.
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

Physical Therapist Requirements & Responsibilities
