Continuum Therapy Partners, LLC is seeking a Physical Therapist to join our team in Front Royal, VA. The role focuses on delivering high-quality patient care by evaluating and treating patients with evidence-based plans, while collaborating with other disciplines to optimize outcomes.

The ideal candidate holds a Doctor of Physical Therapy, an active state license (NPTE) and has at least 1 year of relevant experience.

• Ability to develop and implement individualized treatment plans

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention

Physical Therapist Requirements & Responsibilities

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
