Continuum Therapy Partners is seeking a Physical Therapist to join our team in Charlottesville, VA. The role focuses on evaluating and treating patients with individualized plans, while collaborating with other disciplines to ensure optimal outcomes.

The ideal candidate holds a DPT, a current Virginia PT license, and has strong patient education skills. A full-time daytime schedule with comprehensive benefits is provided at Hillsdale Health and Rehab in Charlottesville, VA.

Qualifications

• Doctor of Physical Therapy (DPT) degree required.
• Current, unencumbered state Physical Therapy License; must pass the NPTE.
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.
