Continuum Therapy Partners, LLC in Dayton, OH is seeking a licensed Physical Therapist to join our patient-centered team. The ideal candidate holds a Doctor of Physical Therapy (DPT), an active state license, and is prepared to sit NPTE.

This full-time daytime role emphasizes evaluating and implementing individualized care plans, coordinating with disciplines, documenting progress, and educating patients and families on home programs.

Qualifications

• DPT degree required.
• Current, unencumbered state PT license; must pass NPTE.
• 1 year of previous experience as a Physical Therapist preferred.
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities
