Job summary

Continuum Therapy Partners, LLC in Dayton, OH is seeking a licensed Physical Therapist to join our therapist-led team. The role requires a Doctor of Physical Therapy (DPT), an active Ohio PT license, and the ability to develop and implement individualized treatment plans.

You will collaborate with other disciplines, document progress, educate patients on home exercise programs, and contribute to a full-time daytime schedule at Wayne Health & Rehab Center in Huber Heights, OH.

Qualifications

• Doctor of Physical Therapy (DPT) degree required.
• Current, unencumbered state Physical Therapy License; must pass the NPTE.
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.
