Job summary

Continuum Therapy Partners, LLC is seeking a Physical Therapist in Pittsburgh, PA. The role emphasizes evaluating and designing patient-specific treatment plans and coordinating care with other disciplines.

Qualified candidates hold a Doctor of Physical Therapy, an active NPTE-eligible license, and preferably 1 year of PT experience. The team values education of patients and families and supports a PT-led care model.

Qualifications

• DPT degree required.
• Current, unencumbered state physical therapy license; NPTE.
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities

• Doctor of Physical Therapy (DPT) degree required
