Continuum Therapy Partners, LLC is seeking a Physical Therapist to join our Milwaukee facilities. You will evaluate and implement individualized treatment plans working with a collaborative care team to improve patient outcomes.

The role requires a Doctor of Physical Therapy, an active state license (NPTE) and offers a full-time daytime schedule with comprehensive benefits, licensure reimbursement, and generous PTO. We value therapists’ voices in shaping care and growth.

Qualifications

• Doctor of Physical Therapy (DPT) degree required.
• Current, unencumbered state Physical Therapy License; must pass the NPTE.
• 1 year of previous experience as a Physical Therapist preferred.
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
