Job summary

Continuum Therapy Partners in Lake City, FL is seeking a qualified Physical Therapist to join our on-site team. This full-time role focuses on delivering evidence-based care in a supportive, therapist-led environment at Lake Montgomery Skilled Nursing & Rehabilitation, Lake City, FL.

You will evaluate patients, develop individualized plans, document progress, and educate patients and families on home exercises and injury prevention.

Qualifications

• Degree from an accredited Physical Therapy program (Bachelor's, Master’s, or Doctorate).
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE).
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.
