Job summary

Flagstar Rehab is seeking a Physical Therapist (PT) to join our skilled nursing facility team in NYC and surrounding areas. You will evaluate patients, create individualized treatment plans, and collaborate with a multidisciplinary staff to improve function and cognition.

You will supervise PTAs, document progress in EMR, and support patient and family education. We offer options for direct employment or as an independent contractor, with competitive hourly pay and weekly pay via direct deposit,

Qualifications

• Current NYS PT license and registration.
• CPR certification.
• EMR competence required.

Responsibilities

• Provide efficient evaluations and create individualized treatment plans.
• Supervise PTAs daily.
• Deliver therapy in SNF with conditions like TBI, CVA, Stroke, Parkinson’s, MS, cancer, and orthopedic surgery.
• Document patient progress in EMR.

Physical Therapist (PT) in Skilled Nursing Facility

Now Staffing Physical Therapists for Rehab Department in state-of-the-art SNFs throughout NYC and Surrounding Areas.

Flagstar Rehab PTs work with a diverse and multi-disciplinary team to perform evaluations, treatments, and patient/family education to improve the physicality and cognition of their patients and positively impact their ability to function in their daily lives.

Flagstar Rehab offers the opportunity for Direct Employment with our agency OR to Be Your Own Boss as an Independent Contractor. We can assist you with setting up a corporation free-of-charge.

Pay Rate: $46-$48 Hourly (W-2), $57-$60 Hourly (1099)

Physical Therapist Job Duties:

• Provide efficient evaluations and create individualized treatment plans for PTAs to follow.
• Supervise PTAs daily.
• Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.
