Job summary

Flagstar Rehab is seeking Physical Therapists (PT) for Skilled Nursing Facilities. We place therapists in positions that fit their schedules with a “choose the way you earn” model and support for independent work if you prefer. Remote onboarding helps a smooth transition between roles while delivering care to patients.

Pay rates: $46-$48 hourly (W-2) or $57-$60 hourly (1099). We offer full-time, part-time, per diem, weekends, days and evenings with onboarding and ongoing support.

Qualifications

• Must hold a NYS PT License and Registration.
• Valid BLS Certification required.
• EMR competence required.
• Meets all State and Federal requirements for US employment.
• Malpractice/Liability Insurance (if applicable).

Responsibilities

• Provide efficient evaluations and create individualized treatment plans for PTAs under your supervision.
• Provide quality therapy services to SNF residents with diverse conditions.
• Monitor and document patient progress using EMR software.

Physical Therapist Requirements and Responsibilities

• Current NYS PT License and Registration
• Valid BLS Certification
• EMR Competence
• Meets all State and Federal requirements for US employment in a Nursing Facility
• Malpractice/Liability Insurance (if applicable)
• Provide efficient evaluations and create individualized treatment plans for PTAs to follow under your supervision.
• Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.
