Job summary

Flagstar Rehab seeks a licensed Physical Therapist (PT) to join our Skilled Nursing Facility team in NYC and surrounding areas. You will perform evaluations, develop treatment plans, and educate patients and families to improve function and cognition in daily activities.

You will work with a multidisciplinary team in a state-of-the-art SNF setting, supervising PTAs, documenting progress in EMR, and delivering high-quality therapy with a focus on rehab outcomes.

Qualifications

• Current NYS PT license and registration required.
• CPR certification required.
• Proficiency with EMR systems.

Responsibilities

• Provide efficient evaluations and create individualized treatment plans for PTAs to follow.
• Supervise PTAs daily.
• Provide quality therapy services in SNF for diverse diagnoses.
• Monitor and document patient progress using EMR.

Physical Therapist (PT) in Skilled Nursing Facility

Now Staffing Physical Therapists for Rehab Department in state-of-the-art SNFs throughout NYC and Surrounding Areas.

Flagstar Rehab PTs work with a diverse and multi-disciplinary team to perform evaluations, treatments, and patient/family education to improve the physicality and cognition of their patients and positively impact their ability to function in their daily lives.

Flagstar Rehab offers the opportunity for Direct Employment with our agency OR to Be Your Own Boss as an Independent Contractor.

Pay Rate: $46-$48 Hourly (W-2), $57-$60 Hourly (1099)

Physical Therapist Job Duties:

• Provide efficient evaluations and create individualized treatment plans for PTAs to follow.
• Supervise PTAs daily.
• Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.
