Job summary

Continuum Therapy Partners, LLC in Alliance, Ohio is seeking a Physical Therapist to join our therapist-led network. The role focuses on evaluating, developing, and implementing individualized treatment plans in coordination with other disciplines, delivering high-quality patient care at Canterbury Villa of Alliance.

You will document patient progress, adjust plans based on outcomes, and educate patients and families on home exercise programs and injury prevention.

Qualifications

• DPT degree required.
• Current, valid state Physical Therapy license; NPTE required.
• One year of experience as a Physical Therapist preferred.

Responsibilities

• Documents patient progress and adjusts plans of care based on outcomes.

Physical Therapist Requirements & Responsibilities

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
