Job summary

Continuum Therapy Partners, LLC is seeking a Physical Therapist to join our team at Menominee Health Services in Menominee, MI. You will evaluate and treat patients with a therapist-led approach and coordinate with the care team to develop effective plans.

Qualifications

• Doctor of Physical Therapy (DPT) degree required.
• Current, unencumbered state Physical Therapy License; must pass the NPTE.
• 1 year of previous experience as a Physical Therapist preferred.
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities
