Job summary

Andwell Health Partners is seeking a Physical Therapist Case Manager to provide high-quality therapy services in clients' homes and communities in Scarborough, ME. The role emphasizes collaboration with families, physicians, and pay sources to promote independence and well-being.

The position requires a PT degree, Maine license, CPR certification, and a driver's license with reliable vehicle. Prior home health experience is preferred, and scheduling offers day shifts with flexibility.

Qualifications

• Bachelor, Master or Doctorate in Physical Therapy from an accredited institution.
• Current PT license in the State of Maine.
• Current CPR certificate required.
• 1-year Physical Therapy experience preferred; home health experience preferred.
• A reliable car with insurance.

Responsibilities

• Provide physical therapy services to clients according to standards of practice outlined by the American Physical Therapy Association, including screening, evaluation, direct service, consultation and/or instruction.
• Coordinate client home care needs in collaboration with client, family, physician, other providers and pay sources toward outcomes promoting client independence.
• Develop, implement, coordinate, and revise a plan of care that aligns with client age, condition and needs.
• Execute care plan activities aimed at promoting independent living for the client and family.
