Job summary

Andwell Health Partners in Farmington, ME is seeking a Physical Therapist Case Manager to provide medical therapy services in clients’ homes and communities. The role supports patients through evaluation, treatment planning, and coordination with families, physicians, and other providers to promote independence and well-being.

This position offers flexible scheduling as part of a part-time or full-time arrangement, requires Maine PT licensure and CPR, and favors home health experience.

Qualifications

• Bachelors, Masters and/or Doctorate Degree in Physical Therapy from an accredited institution.
• Current PT license in the State of Maine.
• Current CPR card required.
• 1-year Physical Therapy experience preferred. Home health experience preferred.
• A reliable car with insurance.

Responsibilities

• Provide physical therapy services to clients according to standards of practice outlined by the American Physical Therapy Association. Services may include screening, evaluation, direct service, consultation and/or instruction.
• Demonstrate knowledge and case management skills to coordinate client home care needs in collaboration with client, family, physician, other providers and pay sources towards outcomes promoting client independence and well-being.
• Develop, implement, coordinate, and revise a plan of care that incorporates goals consistent with the client’s age, condition, and needs.
• Implement client care plan activities based on treatment goals towards independent living for the client/family.
