Andwell Health Partners seeks a licensed Physical Therapist Case Manager to deliver PT services in clients' homes and community settings in Scarborough, ME. You will assess, plan, and coordinate care with families and healthcare providers to promote independence and wellbeing.

Board-eligible or licensed PT with CPR certification is required; home health experience and reliable vehicle are preferred. Flexible scheduling and comprehensive benefits accompany this role.

What You'll Do

• Provide physical therapy services to clients according to standards of practice outlined by the American Physical Therapy Association. Services may include screening, evaluation, direct service, consultation and/or instruction.
• Demonstrate knowledge and case management skills to coordinate client home care needs in collaboration with client, family, physician, other providers and pay sources towards outcomes promoting client independence and well-being.
• Develop, implement, coordinate, and revise a plan of care that incorporates goals consistent with the client's age, condition, and needs.
• Implement client care plan activities based on treatment goals towards independent living for the client/family.

What You'll Bring

• Bachelors, Masters and/or Doctorate Degree in Physical Therapy from an accredited institution.
• Current PT license in the State of Maine.
• Current CPR card required.
• 1-year Physical Therapy experience preferred. Home health experience preferred.
• A reliable car with insurance.
