Andwell Health Partners seeks a licensed Physical Therapist Case Manager to deliver PT services in clients' homes and community settings in Scarborough, ME. You will assess, plan, and coordinate care with families and healthcare providers to promote independence and wellbeing.

Qualifications

• PT degree (Bachelor/Master/Doctorate) in Physical Therapy from an accredited program.
• Current Maine PT license.
• CPR certification required.

Responsibilities

• Provide physical therapy services per APTA standards.
• Coordinate home care needs with client, family, physician and providers.
• Develop and revise a plan of care aligned with patient goals.
• Implement plan to promote independent living.
